If in 2022, you
– Made contributions to the Vexty Plan.;
– Made a withdrawal;
– Received monthly income benefit or lump sum benefit.;
– Separated from the sponsoring company and did not make any fund transactions.;
– Made an inbound or outbound portability transfer.;
– On December 31, 2022, held a loan agreement with Vexty.
